Policy on Accounting of Disclosures

Covered entities are required to track disclosures of protected health information (PHI) in order to provide a patient or legal representative with an account of disclosures for the 6 (six) years prior to the date of their request (164.528) or if the patient/legal representative specifies a period of time less than the 6 six years prior to the request date. The accounting must include disclosures to or by business associates.
